NEW PATIENT

Welcome

New patients that schedule for an initial evaluation at Telemental will receive two emails.

​

1) Email from Telemental including new patient intake forms that must be completed at least 24 hours prior to scheduled appointment.

2) Email from Headway "Team Headway" including billing and insurance information in addition to patient disclosure forms that must be completed 24 hours prior to scheduled appointment.

Telemental uses Headway for the management of patient billing, insurance, an administrative needs.

This is what can be expected from Headway

1) Invitation email to obtain patient insurance information, payment, and disclosure forms. 

2) Ahead of each appointment, clients will receive appointment reminders and estimated cost.

3) Appointments will be charged to payment method on file, client will receive invoice â€‹to email linked.
